Enhanced eBooks include interesting features like embedded media (audio and video), interactivity, audio book (Media Overlay/narration), and more. Both reflowing and fixed layout ebooks can be enhanced (or not).

Both ePub and Amazon formats can be enhanced, although support for enhanced Amazon formats is limited to certain devices and apps, so it can complicate the sales and marketing of such books.
An enhanced ebook adds great value to your product, but it also comes with some extra challenges, particularly when it comes to device support and distribution. Unfortunately the publishing industry is still plagued by format wars and licensing complications that mean not all devices can support enhanced features, such as audio and video.

We will also advise you if we feel your interactive ebook plans are too complex and you should instead consider developing an app. Ebook enhancements are multimedia features that are limited to specific eBook devices and apps. Apple iBooks currently has the best support for enhanced ebooks. Kobo supports Media Overlay (read more about this on the FAQ).
We started using Amazon’s Interactive Ebook multimedia text book format in 2016 with Amanda Greenslade's children's book Myra and the Magic Motocycle.
The first thing to determine for yourself is if you think your readers really would want to fill in the forms you mentioned on their iPad. Currently Apple iPad is the only device and ebook vendor supporting fillable forms.

While we are keen to work on an Apple iBooks Author project, with forms and other interactive features, this program is so easy to use that we find most authors DIY. It may take a fair amount of time, but it is a good way to work, if you are the creative visual/design type of writer. We often come in more towards the end with one of these projects where our customers have encountered a hurdle (eg. with video formats or distribution).
